This is a Direct Hire Solicitation
Conduct clinical assessments of patients health needs to formulate individualized, patient-centered care plans.
Educate patients, their families, and the healthcare team on treatment options, insurance benefits, and available community resources.
Arrange necessary health services, specialized referrals, and follow-up appointments across both military and civilian sectors.
Analyze complex cases to identify barriers or systemic problems related to healthcare access and utilization.
Review medical referrals for clinical accuracy and assist in routing patients to the appropriate specialty providers.
