The Intake/Aftercare Coordinator provides assessments, intake, discharge planning, aftercare, and clinical social work services for American Indian and Alaska Native youth ages 12-17 in residential treatment for substance use and co-occurring conditions. The position works with Tribal programs, families, referral sources, and treatment teams to support continuity of care and reports to the Clinical Director or designee.
Coordinates referrals for youth requiring residential behavioral health treatment, including substance use disorders and co-occurring behavioral health conditions, ensuring timely admission and continuity of care. Coordinates the intake and admission process by reviewing referrals, ensuring required documentation is complete, communicating with referral sources, families, Tribal programs, and interdisciplinary treatment team members, and facilitating timely admission, treatment planning, discharge planning, and aftercare coordination. Conducts comprehensive biopsychosocial and behavioral health assessments to evaluate substance use disorders, co-occurring behavioral health conditions, psychosocial functioning, strengths, risk factors, and treatment needs.
Provides clinical assessment, triage, consultation, crisis intervention, and care coordination for youth and families presenting with substance use disorders, co-occurring behavioral health conditions, and related psychosocial needs. Develops individualized discharge and aftercare plans, coordinates continuity of care with Tribal and community providers, facilitates referrals for ongoing behavioral health services, and conducts post-discharge follow-up to support successful community reintegration. Develops and maintains collaborative relationships with Tribal governments, Urban Indian Organizations, behavioral health providers, schools, courts, and community partners to support outreach, education, referral development, and continuity of care.
