Job description

This position is the Liaison/Case Manager for VA Central California's Grant & Per Diem (G&PD) Program and is responsible for monitoring and coordination of services provided by community-based programs funded under VA's Homeless Providers Grant and Per Diem Program. The liaison is the key point of contact between the programs in the field and the National G&PD Program Office. They will provide the oversight of these services as well as the case management of the Veterans who receive them.

Major duties include: Develops and implements the GPD Program specific to his/her area of assignment and insures compliance and collaboration with local facility and the VA Central Office initiatives. Administers information and analytical systems to evaluate and enhance the quality of service provided to Grant recipients. Develops and implements information systems to track service activities including visits, referrals, demographics, equipment utilization and Veteran satisfaction.

Coordinates and oversees all required GPD program inspections to ensure provider compliance with VA regulations, life-safety standards, and grant agreement requirements. Schedules and conducts annual, initial, and follow-up inspections in collaboration with interdisciplinary team members an GPD grantees. Accurately records inspection findings, issues reports within required timelines, and monitors completion of corrective actions to maintain program integrity and Veteran safety.

Maintains detailed inspection records to support accountability, audit readiness, and high-quality services for Veterans experiencing homelessness. Exercises strong fiscal responsibility in the administration and oversight of the GPD Program. Ensures all GPD funding is utilized in accordance with the VA regulations, grant agreements, and federal fiscal policies.

Monitors provider budgets and expenditures to ensure allowable use of funds, identifies discrepancies or risks, and collaborates with GPD grantees to reinforce compliance and promote sound financial practices. Maintains accurate, timely documentation related to payment requests, inspections, corrective action plans, and program audits. Communicates proactively with leadership and community partners regarding fiscal needs, spending trends, and resource gaps to support the sustainability and effectiveness of GPD services for Veterans experiencing homelessness.

Provides referral to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.
