Join the VA Black Hills Health Care System at the Rapid City Community Based Outpatient Clinic as an Outpatient Staff Registered Nurse. In this rewarding role, you will deliver Veteran-centered primary and specialty care, coordinate services across the care continuum, promote wellness and chronic disease self-management, provide education and coaching, administer treatments, and collaborate with an interdisciplinary team to improve outcomes and the patient experience.

As an Outpatient Staff Registered Nurse, you will partner with Veterans, families, caregivers, and interdisciplinary professionals to develop individualized care plans that reflect each Veteran's needs, preferences, and health goals. This role offers the opportunity to strengthen clinical judgment, expand primary and specialty care expertise, and help Veterans achieve greater independence through wellness coaching, disease prevention, and effective chronic care management. Duties include, but are not limited to: - Assess Veterans' health needs and develop individualized nursing plans based on clinical findings and personal goals of care. - Deliver and evaluate evidence-based nursing care within primary and specialty outpatient settings.

- Coordinate care across the continuum to promote timely services, continuity, and positive health outcomes. - Educate and coach Veterans, families, and caregivers on wellness, disease prevention, self-management, and chronic condition management. - Administer medications, treatments, and procedures in accordance with established clinical guidelines.

- Collaborate with interdisciplinary team members to develop, implement, and evaluate comprehensive care plans. - Engage Veterans and their support systems in shared decisions that incorporate their values, preferences, and beliefs. - Delegate appropriate elements of care based on the Veteran's assessed needs, clinical condition, and safety considerations.
