The Clinical Resource Hub (CRH) Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Urgent Care (UC) Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. **See note below in duties section regarding location of position.**
Location Clarification: This job is open to candidates who reside and/or intend to reside within a 50-mile radius of a facility located within the VISN 5 catchment area: Alaska, American Samoa, Arizona, California, Guam, Hawaii, Idaho, Montana, Nevada, New Mexico, Northern Marina Islands, Oregon, and Washington. General Description of Assigned Duties: Responsible for managing patients with acute and/or chronic illness in the Clinical Video Telehealth setting as part of the VISN Clinical Resource Hub. Provides clinical management for a variety of complex health conditions.
Serves all Veterans Affairs facilities within a Veterans Integrated System Network (VISN). Provides interprofessional services to VISN facilities that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Practices to the full extent of their education, training, and certification. Prescriptive authority for a panel of patients or program as indicated by their position.
Functions or Scope of Assigned Duties: Supports the VISN CRH under the guidance of the Veterans Health Administration (VHA) which provides health care services to Veteran patients across the continuum of care in both institutional and non-institutional settings. Provides Telehealth not only to clinical areas within the VISN CRH but also other VISN CRHs throughout the nation. Services provided by CRH staff are conducted via multiple modalities.
Major duties and responsibilities of the position: Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Urgent Care Team members, Primary Care Teams and other interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies.
Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families.
Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. Is proactive in identifying potential emergency/crisis situations and providing appropriate interventions.
Documents in Computerized Patient Record System (CPRS). Performs additional duties as assigned to meet the needs of CRH.
