Job description

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Former EDRP participants ineligible to apply.

Located in the Rogue Valley, White City allows easy access to Metro-area of Medford while remaining engrossed with nature. White City is an unincorporated community in Jackson County, Oregon, in the Rogue Valley. It is about nine miles north of Medford and 34 miles north of the California border.

Take a family-friendly hike to a waterfall or ride the only section of the Pacific Crest Trail that's open to mountain bikers. The Mental Health Residential Rehabilitation Treatment Program Nurse Practitioner independently manages a Veteran patient panel in a residential mental health program, providing comprehensive evidence-based primary care, health promotion, disease prevention, and chronic condition management. The NP leads interdisciplinary care planning, orders and interprets diagnostics, prescribes pharmacologic and non-pharmacologic treatments, coordinates referrals, and adjusts plans of care to optimize outcomes while modeling ICARE values and driving quality-improvement initiatives. Duties include, but not limited to: Independently manages Veterans in RRTP with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.

Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies.

Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
