Nurse Case Management (PRC)

This position is located in the Purchased/Referred Care (PRC) department at Northern Cheyenne Service Unit, Lame Deer, Montana. Responsibilities include providing clinical case management for the PRC Program by coordinating healthcare services among inpatient and outpatient care teams, external healthcare providers, and the PRC Program. The incumbent works under the direction of the PRC Supervisor.

Facilitates referrals according to medical priorities and patient needs. Manages and triages case management consult request.

Participates in care team meetings as an advocate for the patient and families. Develops patient care and service delivery plan with interdisciplinary teams. Serves as a resource on case management/managed care issues for service unit.
