Serves as an independent VA Advanced Practice Nurse/Nurse Practitioner in Gastroenterology/Hepatology and Specialty Care, managing patients with acute and chronic conditions. Provides evidence-based assessment, diagnosis, treatment, prescribing, referrals, education, and follow-up; orders and interprets tests; collaborates across disciplines; leads quality improvement; supports staff growth; protects sensitive data and adapts to patient needs, physical demands, and flexible scheduling.
Independently manages a panel of patients with acute and chronic health conditions. Provides evidence-based care focused on health promotion and disease prevention. Performs advanced clinical assessments and evaluates patient health status.
Develops and implements individualized plans of care. Diagnoses and manages complex clinical conditions within approved privileges. Orders and interprets laboratory tests, diagnostic studies, and other clinical evaluations.
Prescribes pharmacologic and non-pharmacologic treatments. Initiates referrals and consults with specialty services. Monitors patient progress and adjusts treatment plans to improve outcomes.
Educates patients and families regarding conditions, treatments, self-care, and prevention. Collaborates with patients, families, and interdisciplinary health care teams. Leads or participates in patient care teams, workgroups, and interdisciplinary initiatives.
Promotes continuity of care and accountability for patient, panel, program, or population outcomes. Applies research, clinical practice guidelines, and evidence-based practice to care delivery. Identifies clinical and operational opportunities for improvement.
Develops, supports, or leads quality and performance improvement initiatives. Evaluates care processes, trends, and outcomes and takes appropriate corrective action. Educates, mentors, and serves as a resource to colleagues, students, and interprofessional staff.
Demonstrates professional leadership, effective communication, ethical conduct, and teamwork. Protects sensitive patient information and complies with privacy, security, safety, and patient-handling requirements. Other duties, as assigned.
