This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
The Homeless Patient-Aligned Care Team (HPACT) is located within the Community Reintegration and Recovery Service (CRRS) of the North Florida/South Georgia VHS. The incumbent will be a part of the multidisciplinary HPACT team as well as a member of the Health Care for Homeless Veterans (HCHV) Outreach Team. All members participate in outreach targeting chronically homeless Veterans wherever they are including but not limited to shelters, soup kitchens, on the streets, in parks, and in rural areas, who are eligible for VA care but have not established ongoing primary care in the VA system.
This position is located in Jacksonville, FL. This Social Worker provides outreach, assessments, referrals, and case management services to homeless Veterans identified in the service areas within North Florida/South Georgia VHS. Case management services are also provided to homeless Veterans receiving treatment in contract residential treatment and supportive housing programs. Veterans served are referred from various homeless shelters, Veterans Services Officers, social service agencies, and from within the VA Medical Center.
Major duties include but are not limited to: Provide support to the VA homeless program teams as a functional member of the team, to include participation in outreach activities and subject matter consultation on community involved interventions for homeless Veterans. Coordinates care, develops and uses VA and community resources to facilitate referrals, develops and implements treatment plans and to provide accurate information to patients, family, staff, and other internal and external customers. Provide all necessary assessment functions in the service provision for homeless Veterans, e.g. interviewing, psychosocial histories and assessments to aid in the development of treatment plans as well as case conferencing discussions and planning.
Uses a high level of skill in assessing and treating complex psychosocial needs of Veterans and their family members/caregivers often during times when the Veteran/family members/caregivers are experiencing crisis. Incumbent requires knowledge of a broad range of psychosocial, medical, and mental health issues. Provides group and individual counseling, and psychotherapy.
Coordinates care, develops and uses VA and community resources to facilitate referrals, develops and implements treatment plans and to provide accurate information to patients, family, staff, and other internal and external customers. Develop partnerships with community agencies with regular contact and communication. Provide appropriate clinical documentation for all contacts with or about Veterans wo are engaged through outreach services. Ensure timely and accurate entry into VA data systems such as HOMES.
Utilizes Government vehicles to provide community outreach including incidental driving and transportation of Veterans as needed based on their treatment needs. Additional Benefits Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
