HUD-VASH is an intensive clinical case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans who reside in stable housing through the voucher program. The HUD-VASH Program aligns with the Health Care for Homeless Veterans (HCHV) Programs within the VASDHS Social Work Service. Nursing supervision is provided by the Nurse Manager HBPC/Telehealth and program expectations are communicated through the HUD-VASH Social Work Supervisors.
The HUD-VASH RN Case Manager contributes clinical expertise for supportive health care interventions to support stable housing and collaborates with the Veteran to identify care goals and coordinate care resources within the VA and Community. The HUD-VASH RN Case Manager assumes the following major duties and responsibilities: 1. Serves as a member of the HUD-VASH Intake Team completing assessments and assisting with coordination of eligibility and resources.
2. Provides direct nursing care by administering assessments, planning and implementing interventions, and monitoring outcomes. 3.
Provides health teaching, health counseling, provision of care supportive to or restorative to life and well-being. 4. Documents patient care assessments, interventions, care coordination, and care plan outcomes in the electronic medical record.
5. Provides nursing care coordination to ensure comprehensive service delivery including referrals to resources/clinical services with the VA San Diego Health care System and/or community agencies. 6.
Plans, schedules, and completes nursing visits in the home, clinic, by phone, or using approved video technologies. 7. Establishes individualized nursing care plan and contributes to the Interdisciplinary Care Plan. 8.
Provides care coordination with the HUD-VASH interdisciplinary team, transportation, community home care agencies, community hospital/providers, RCFE, skilled nursing facilities, and homeless shelters. 9. Maintains a panel of Veterans requiring ongoing chronic disease management and nursing intervention.
10. Completes HUD-VASH VA Grant and Per Diem clinical inspections at assigned locations. 11.
Provides nursing medication management, education, and monitoring. 12. Provides leadership in unit performance improvement, HRO, and system redesign initiatives.
13. Participates on local, VISN, and/or national HCHV HUD-VASH committees, councils, or workgroups. 14.
Develops/Updates unit-based policies and standard operating procedures. 15. Maintains valid and unrestricted driver's license for travel to Veteran homes, community outreach activities, and various VASDHS locations during both day and night time tour of duty hours.
16. Serves as preceptor or clinical resource for nursing students and interdisciplinary HUD/VASH team. 17.
Performs other duties as assigned.
