Nurse (HBPC Outpatient)

The Nurse Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex HBPC care. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program.

Bend is a city in Central Oregon and the county seat of Deschutes County, Oregon, United States. It is located to the east of the Cascade Range, on the Deschutes River. The site became known by pioneers as a fordable crossing point of the river, where it was ran through a bend.

An 1870s ranch popularized the name "Farewell Bend", with the post office later distinguishing the area as Bend. It was incorporated as a city in 1905, starting off as a logging town. In 1910, Mirror Pond was created as a dammed river reservoir to provide energy.

In 1950, the two major logging companies were consolidated due to depleted timber, causing an economic drop. In later decades, it experienced rapid growth as a center of recreation. Salem is a mid-size city in the Willamette Valley and is part of Oregon's second-largest metropolitan area.

It is known as the home of the Oregon State Fair and for its beautiful scenery and access to nature. Situated between the Cascade Mountain Range and the Pacific coastline, Salem is a great location for outdoor adventures such as visiting old-growth forests, scenic waterfalls, and coastal tide pools. The city also boasts unique nightlife, shopping, and colorful community events for those who enjoy urban and city living.

Major duties include, but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.

Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.

Makes home visits to Veterans in the program. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for the HBPC Program and facility.

Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through HBPC knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals.

Evaluates outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations. Responsible for the documented HBPC outcomes at the program or service level.
