Advanced Practice Nurse - Geriatrics and Extended Care

The Nurse Community Living Center Admission Coordinator Advanced Practice Nurse (APN) is responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.

Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention, maximization of function, and/or assessing and honoring seriously ill Veteran's goals and preferences for care and quality of life using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership - Works collaboratively multi-disciplinary team Orders and interprets diagnostic studies - Communicates effectively with patients, patient caregivers, colleagues, and interdisciplinary team members - Cooperates with nursing staff and other disciplines to provide patient care Prescribes pharmacologic and non-pharmacologic interventions.
